Below are forms you will need if you are working in DE or have an idea for DE.  Some of the forms are in Word format; feel free to complete them on the computer.  If a signature is required, please feel free to type the necessary information then print the form off and sign it.  Remember, keep a signed copy for yourself in case a form is misplaced.
